How does the Support at Home Program differ from Home Care packages?
Under the new November 1st 2025 Support at Home program, there is new access to services, products, equipment, and home modifications to help older adults remain healthy, active, and socially connected to their communities. Some key changes from HCP:
-Moving to a quarterly budget system
-Introduction of a defined service list
-Care management funding set at 10% of all quarterly budgets

What if I need extra help for a short-term to help me get back on my feet?
Commonwealth Home Support Program and Home Care Package will be able to access two short-term care pathways funded under Support at Home. You will need to be assessed by Single Assessment Team to access the following:
Restorative Care Pathway, to help you maintain and improve your independence through multidisciplinary allied health services.
End-of-Life Pathway, to access additional services and remain at home for up to 16 weeks.
Can I take a break from my services under Support at Home
You have the flexibility to temporarily suspend your Support at Home services for various reasons (e.g., a hospital stay or holiday). While services are stopped, you will continue to receive your quarterly budget. However, carry-over unspent budget limits will apply.
Keep in mind that your funding will be reduced to zero and reallocated once four consecutive quarters (one year) and 60 days have passed since the end of the quarter in which the last service was delivered.
