How Does it work?
Step 1
You or your family member has decided that extra support at home is required. Contact the Australian Government's My Aged Care (or Department of Veterans' Affairs for VHC) to check your eligibility and organise an assessment.
Step 2
They'll assess your needs and determine the support you need and level of funding you're eligible for. You'll receive a letter with the outcome of your assessment, including your unique referral code.
Step 3
Use My Aged Care to find a local provider. Contact our friendly team to discuss the services you require.
Step 4
Our friendly care Co-ordinator will get in touch to develop a customised care plan to suit your unique needs.
